New Patient Information
Anderson Integrative Medicine is located at 3031 S Russell, across the street from the YMCA. Look for the sign that says “Health Options” and park on the south side of the building, entering through the side door.
Please note, while the unpredictability of patients’ needs will sometimes cause delays in the office, we try very hard to respect your time and stay on schedule. We ask that you please arrive on time for all appointments. If you are going to be late, please call and notify our office as soon as possible.
Please print and complete all of the forms below and bring with you, arriving 10 minutes early for your appointment. If you would prefer to complete your paperwork at the office, please arrive 20 minutes early.
